I just got my new passport from ROC with the "Also known as" field
as "TE, KHAI-SU". Birthplace is now "TAIPEI CITY" instead of "CHINA",
and the new passport is now machine readable rather than the old one.
How did I make the change?
Also known as
-------------
First you have to apply for a "World Identity Card" from the "World
Service Authority" with your name spelled in Peh-oe-ji.
World Service Authority is a service unit under the World Government
of World Citizens. World Government think that all people are World
Citizens. However, you still have to register with them. They issue
documents such as the "World Passport", "World Identity Card",
"International Exit Visa", as a service to the oppressed.
You can write them asking them to send you the application. After you
received the application, fill in your name spelled in Peh-oe-ji in
all "name" fields. You have to get the "World Identity Card", but you
can also get some other documents from them. Then, enclose your proof
of identity. An ROC identity card will suffice. Enclose a letter
with your application explaining that your Han character name will
be pronounced as indicated by the Peh-oe-ji spelling. If you are
applying for the Passport, since it is handwritten, diacritics can be
represented. However, the Identity Card is typeset, so please indicate
in your letter your TP 1a4 representation (no diacritics, all caps),
for example:
KHOU, KUN-LAM
LI, KHIN-HOANN

The application process will take about 2 weeks.
Then you have to deal with the ROC bureaucracy. When you apply
for the ROC passport, fill in the "Also known as" section with
your Peh-oe-ji name, in all caps, without diacritics, for example:
KHOU, KUN-LAM
LI, KHIN-HOANN
In the "Document for proof" field of the "Also known as" section,
fill in "World Identity Card". Show the officer your "World
Identity Card" with your Peh-oe-ji name. If they ask for a
photocopy, give them a photocopy. To avoid trouble, try not to
show them your World Passport.
Your new passport will have your Peh-oe-ji name in the "Also known
as" field.
>From now on, I suggest you use the Peh-oe-ji name as your primary
name, and fill in the "Also known as" field with your Mandarin name;
essentially, do the opposite as on the passport.
Birthplace
----------
If you feel uncomfortable about that your passport list your
birthplace as "CHINA", and want to change it to your real birthplace:
This is easier. Change your ROC identity card so that it lists
your real birthplace instead of your "ancestral origin". Then apply
for a new passport with the new identity card, and you are all set.
Conclusion
Don't think that once this is done, you have nothing else to put
your efforts to! Traveling with the low-class ROC passport is a
shameful thing to do! It is not a good thing to deal with this
"Republic of China"! We shall establish our own country so we
will be traveling with the Passport of the Taiwan Republic with
pride and the International Community's respect!
